Family Empowerment Coach

<p> </p><p>Classification: Full-time </p><p>Reports To: Program Director</p><p>Position Overview:</p><p>Under the general supervision of the Program Director, the Family Empowerment Coach (Case Manager) will establish a positive relationship with Almost Home clients in order to support them in making the necessary behavioral and attitudinal changes that will allow them to successfully achieve the program outcomes and individual goals. Responsibilities include providing case management, support, and working closely with the program staff to ensure the delivery of services to clients in the transitional housing programs. </p><p>Core Functions</p><p>o Review of potential members to determine placement fit</p><p>o Administer assessments </p><p>o Provide support for member with obtaining official documents, access childcare and other to </p><p>resources</p><p>o Provide case management in alignment with Member’s Individualized Service Plans and Transition </p><p>Plans based on Almost Home’s Continuum of Care Model</p><p>o Coordinate health/dental care services </p><p>o Work with member to develop long and short-term goals</p><p>o Review goal progression/action step completion during weekly case management sessions</p><p>o Work with community partners to establish and maintain relationships</p><p>o Utilize database(s) to document all case management activities and maintain client records in </p><p>accordance with all state, local and grant funding requirements along with Almost Home within </p><p>48hrs</p><p>o Ensure that Motivational interviewing, solution-focused and trauma informed approaches are </p><p>maintained in client interactions</p><p>o Complete intake assessment utilizing Arizona Self Sufficiency Matrix (ASSM). Create goal-oriented </p><p>actions plan with family. Engage with each active family at least two hours a month to provide </p><p>accountability, resourcing, ongoing coaching, and support of sustainable results. </p><p>o Provide budget counseling for enrolled families and coordinate financial empowerment activities</p><p>o Maintain client confidentiality as well as safety and ethical standards </p><p>o Work alongside other program staff to coordinate client care</p><p>o Ensures that Motivational interviewing, solution-focused, and trauma-informed approaches are </p><p>maintained in client interactions</p><p>o Attends staff meetings, program in-services, Continuum of Care (COC) meetings and client related </p><p>events as needed</p><p>o Maintains reporting for St. Louis County Children’s Service Fund, United for Families and </p><p>Affordable Housing </p><p>o Reports monthly member highlights</p><p>o Manage program scheduling to include life skill’s classes and other events</p><p>o Monthly, Quarterly, Annual tracking, reporting and ensuring member outcome data</p><p>Core Competencies:</p><p>Rev 10/7/2024</p><p>o Customer Oriented o Continuous Learning</p><p>o Professionalism o Ethics & Integrity</p><p>o Promoting Health & Safety o Problem Solving</p><p>o Creativity & Innovation o Teamwork/Collaboration</p><p>o Communication </p><p>Qualifications:</p><p>o Bachelor’s degree in social work, human services, psychology or related field preferred. Master’s </p><p>degree preferred.</p><p>o Minimum of three years’ work experience in a residential setting preferred</p><p>o History of experience providing case management services, preferred</p><p>o At least two years’ experience working with women and children desired</p><p>o Proficient in use of technology including email and Microsoft Office software</p><p>o Excellent written, verbal, and interpersonal communication skills</p><p>o Belief in team approach and proactive in sharing information with others.</p><p>o High level of organization, initiative, attention to detail and follow-through</p><p>o Experience working with hard to reach and at-risk individuals and families</p><p>o Ability to exhibit emotional control, patience and persistence in stressful situations</p><p>o Cultural competency and ability to establish positive rapport with clients</p><p>o Able to manage confidential information with discretion and tact </p><p>o Valid driver’s license and proof of insurance; must obtain Chauffer’s license within 30 days of </p><p>employment</p><p>o Must pass a first aid and CPR course and receive certification in administering medicines within 30 days </p><p>of hire</p><p>Physical Requirements: </p><p>o Walking, standing, bending, stooping, kneeling, and lifting up to 40 pounds</p><p>Work Conditions & Environment:</p><p>o Inside environmental conditions typical of office or administrative work</p><p>o Ability to work in a high-intensity environment with measureable accountability</p><p>o Comfortable working in a dynamic environment and adaptable to change</p><p>o Ability to be flexible in work hours. (1) Saturday a month 8am-4pm, (1) Late evening a week 11am-7pm</p>
